About the Role

We are seeking a skilled Crisis Worker to join our Mobile Crisis Response Team at Ascension Presence Health Care. As a Crisis Worker, you will play a vital role in providing compassionate care to individuals in crisis.

Key Responsibilities
  • Assess clients' social services needs and develop personalized service plans.
  • Conduct initial intake procedures and assess eligibility for services.
  • Participate in the initial assessment process, service plan development, and ongoing monitoring of client care.
  • Resolve client problems by clarifying issues, determining causes, and selecting effective solutions.
Requirements
  • Licensure as a Social Worker from the Illinois Department of Financial and Professional Regulation is preferred.
  • High School diploma equivalency with 2 years of cumulative experience or Associate's degree/Bachelor's degree or 4 years of applicable cumulative job-specific experience is required.
