Job Summary

We are seeking a highly skilled and compassionate Outpatient Therapist to join our team at Communicare. As a key member of our treatment team, you will provide evidence-based services to individuals in their residences or community settings, promoting continuity of care and supporting their mental health needs.

Responsibilities
  • Provide high-quality, individualized services to clients, ensuring fidelity to established treatment plans and promoting positive outcomes.
  • Collaborate with clients, families, and other healthcare professionals to develop and implement comprehensive treatment plans.
  • Conduct thorough assessments, evaluations, and screenings to identify client needs and develop effective interventions.
  • Develop and maintain strong relationships with clients, families, and community partners to ensure seamless care coordination.
  • Stay up-to-date with industry best practices, research, and regulatory requirements to provide the highest level of care.
  • Participate in regular supervision, training, and professional development opportunities to enhance clinical skills and knowledge.
  • Meet productivity and documentation standards, ensuring timely and accurate service delivery.
  • Work collaboratively with other Communicare programs to ensure continuity of care and a coordinated system of services.
Requirements
  • Master's degree in a related field (counseling, psychology, social work, etc.) and licensure or certification in good standing.
  • Ability to work independently and as part of a team, with excellent communication and interpersonal skills.
  • Strong analytical and problem-solving skills, with the ability to think critically and creatively.
  • Valid driver's license, reliable transportation, and a clean driving record.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
