The Gerontology Nurse Practitioner/Physician Assistant at Brio Living Services plays a critical role in delivering comprehensive care to our participants in the PACE program. This is an exciting opportunity for a senior healthcare professional to join our team and contribute to our mission of preserving health, independence, and quality of life for our aging population.
Responsibilities:- Assess and address the needs of participants through comprehensive evaluations and assessments
- Develop and implement individualized care plans in collaboration with the IDT
- Provide education and support to participants and families on chronic conditions and self-management strategies
The APP will also be responsible for collaborating with community partners to ensure seamless care transitions, participating in quality improvement initiatives, and providing on-call support as needed. A Master's degree in nursing or satisfactory completion of a physician assistant program is required, along with BLS certification, current DEA registration, and State of Michigan Controlled Substance License.
Minimum of 1-year experience with the frail and elderly or long-term care population is preferred. We are committed to upholding the organization's values and contributing to a positive workplace culture.
