Job Summary

We are seeking a highly skilled Clinical Care Coordinator to join our team at BayMark Health Services. As a Clinical Care Coordinator, you will play a critical role in ensuring the highest level of care for our patients.

Key Responsibilities
  • Quality Care Standards: Maintain and uphold quality care standards in accordance with BayMark Health Services' policies and procedures.
  • Patient Treatment Monitoring: Monitor patient treatment progress and ensure accurate and up-to-date clinical records.
  • Program Reviews and Counseling: Conduct internal program reviews, patient records reviews, and provide educational counseling on substance abuse disorders.
  • Staff Training and Development: Assess training needs of clinical and support staff and report to the Program Director.
  • Regulatory Compliance: Ensure staff compliance with Federal and State regulations governing patient treatment.
  • Professional Development: Attend conferences, meetings, and training programs as needed to stay updated on current substance use disorders and treatment information.
  • Patient Support Services: Provide intake and readmission services, follow-up calls, and crisis intervention support to patients.
  • Documentation and Reporting: Prepare interagency reports and documentation, and perform monthly file reviews for quality control.
  • Chart Monitoring: Maintain a chart monitoring system to ensure accurate and up-to-date patient records.
  • Community Relations: Participate in community and public relations activities to promote BayMark Health Services' mission and values.
  • Financial Management: Collect patient program fees and manage clinic finances in accordance with BayMark Health Services' policies and procedures.
  • Team Leadership: Develop professional relationships with patients and function as a team leader to promote a positive and professional work environment.
  • Professional Image: Project a positive and professional image to the community at all times.
  • Training and Development: Participate in monthly Clinic Inservice Trainings to stay updated on current best practices and treatment modalities.
  • Follow-up Calls: Provide follow-up calls to new patients to ensure a smooth transition into treatment.
Qualifications
  • Education and Experience: LADC with a Master's degree in behavioral health, and/or LCMHC required. At least five years of experience in the behavioral health field, with one year of experience in administering a substance abuse treatment agency.
  • Certifications and Background Check: Satisfactory background check and any other certifications required by BayMark Health Services.
