Job Summary:The Registered Nurse Case Manager will be responsible for coordinating patient care and managing clinical and financial outcomes for an assigned caseload of patients. This role requires strong communication and collaboration skills, as well as the ability to work effectively with patients, families, and healthcare professionals.
Responsibilities:- Conduct comprehensive assessments of patients' clinical, psychological, and financial needs.
- Develop and implement individualized care plans to address patients' needs and promote optimal clinical outcomes.
- Coordinate and facilitate patient care transitions between healthcare settings, ensuring seamless and efficient care delivery.
- Collaborate with healthcare professionals to ensure accurate and timely documentation of patient information.
- Monitor and analyze patient data to identify trends and opportunities for improvement.
- Develop and implement strategies to improve patient satisfaction and outcomes.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.
- Current licensure as a Registered Nurse (RN) in the State of Oklahoma or current multistate license from a Nurse Licensure Compact (eNLC) member state.
- 5 years of experience in a variety of clinical settings, including home health, inpatient, and physician office settings.
- Experience with managed care and payer/provider requirements preferred.
- BSN preferred.
- Excellent interpersonal, communication, and collaboration skills.
- Computer experience required, with proficiency in Windows and electronic health records.
