Key Responsibilities:

  • Develop and implement patient care plans in collaboration with our interdisciplinary team
  • Provide skilled nursing care to patients in their homes, promoting independence and well-being
  • Collaborate with our care team to ensure seamless transitions and continuity of care
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction

Requirements:

  • RN license in the state you work
  • Graduate from an approved school of practical nursing
  • One year of RN experience and the clinical competence to deliver quality patient care
  • Current driver's license and ability to spend ~20% of your day driving to/from patient locations
  • A commitment to consistently meet critical deadlines for charting
  • The skills needed to self-manage your time and schedule
  • Demonstrated experience with tablets, mobile phones, and EMR software
