Job Title: Medical Coordinator

At Illumination Foundation, we are dedicated to disrupting the cycle of homelessness by providing targeted, interdisciplinary services in our recuperative care centers, emergency shelters, housing services, and children's and family programs.

Job Summary

The Medical Coordinator is responsible for delivering high-quality medical care and influencing positive outcomes for patients experiencing homelessness in the recuperative care setting. We have multiple schedules available for this role, including weekends and nights.

Responsibilities
  • Perform initial intake to ensure an appropriate care plan is established based on discharge information from the hospital or primary care notes.
  • Assess the health status of patients upon arrival by conducting head-to-toe assessments and checking vitals.
  • Determine patients' insurance status, make follow-up appointments with patients' providers, and review medication lists.
  • Establish and maintain care plans, set weekly goals for patients to complete to assist with medical or social reintegration, verify goal completion, and assess medical needs.
  • Conduct morning, afternoon, and evening checks for all assigned clients to observe well-being, ensuring there is no change in medical conditions since hospital discharge.
  • Link clients to medical services when necessary (insurance, PCP, specialists, referrals).
  • Coordinate home health visits (if needed) and follow-up on medical appointments.
  • Promote health literacy by providing patient education regarding clients' illnesses, medications, preventative measures, treatment management, and overall general health.
  • Arrange transportation for clients to and from medical appointments, as needed.
  • Report emergency situations to the Supervisor/Manager and document incidents.
  • Seek proper levels of care to assess and treat medical complications.
  • Initiate medical response systems when appropriate.
  • Support patient recuperation by providing necessary items and space needed to rehabilitate and assisting with the upkeep of the facility.
Qualifications
  • Current and non-restricted LVN license in California.
  • 2 years of relevant experience.
  • High School Diploma or equivalent.
  • Basic computer skills, including the ability to send and receive emails and summarize data in spreadsheets.
  • Valid CA Driver's License.
