Join Our Team as a Physical Therapist in Outpatient Home-Based Care

Position Summary:

We are seeking a skilled Physical Therapist to provide outpatient physical therapy services in the patient's place of residence, utilizing a reliable transportation system. The ideal candidate will evaluate patient function, select and apply appropriate treatments to improve or restore function, and document patient progress in accordance with insurance guidelines and departmental policies.

Key Responsibilities:

  • Evaluate patients based on departmental standards and establish individualized, measurable goals for each patient.
  • Conduct and modify evaluations based on patient symptoms, level of activity, and medical status.
  • Treat patients with therapeutic exercises, physical agents, and manual techniques to relieve pain and attain established goals.
  • Document patient progress and adhere to departmental policies and procedures.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care.

Requirements:

  • Graduate of an accredited Physical Therapy program with a minimum of a B.S.
  • Current Connecticut license in Physical Therapy.
  • CPR certification.
  • 3 years of recent outpatient physical therapy experience preferred.

Competencies:

  • Strong English communication skills, both verbal and written.
  • Personal computer skills and proficiency in Windows-based programs.
  • Basic keyboarding skills and familiarity with Microsoft Outlook, Word, Excel, and internet search capabilities.
