Clinical Patient Care Coordinator

About the PositionIn this role, you will be responsible for assessing patient needs, developing individualized care plans, and delivering high-quality patient care. You will work closely with our multidisciplinary team to ensure the best possible outcomes for our patients.
Key Responsibilities- Assess and evaluate patient needs, developing individualized care plans to meet their unique requirements.
- Provide direct patient care, including administering medications, performing treatments, and providing education to patients and families.
- Collaborate with healthcare professionals to coordinate care, manage patient flow, and optimize resource utilization.
- Maintain accurate and comprehensive medical records, adhering to regulatory standards and facility policies.
To qualify for this role, you must possess a current, valid State Registered Nursing License and CPR Certification. A minimum of one year of clinical experience is preferred, with correctional nursing experience being a plus.
What We Offer- A competitive salary range of $73,000 - $94,000 per year, depending on experience.
