Key Responsibilities- Provide primary care to patients in skilled nursing facilities, including managing chronic disease, diagnosing illnesses, and implementing plans of care.
- Order and interpret lab results, prescribe and monitor medications, and consult with physicians and other healthcare practitioners as needed.
- Build relationships with patients, their families, and facility staff to facilitate effective treatment delivery and promote healthy lifestyle changes.
- Collaborate with the healthcare team to ensure seamless care transitions and prevent avoidable readmissions.
- A minimum of 1 year of experience as a Nurse Practitioner in a primary care setting.
- Active and unrestricted Pennsylvania NP license and DEA with prescriptive authority.
- Post-acute care experience a plus, but not required.
