Responsibilities:
  • Provide comprehensive nursing care to clients in their homes, including medication management, wound care, and treatments.
  • Conduct thorough assessments of clients' needs and develop personalized care plans to meet their unique requirements.
  • Collaborate with clients' healthcare teams to ensure seamless care coordination and communication.
  • Document all client interactions and care provided in a timely and accurate manner.
Requirements:
  • Current licensure as a Registered Nurse in the state of Virginia.
  • Minimum of one year of recent, verifiable clinical experience.
  • Prior home care experience is strongly preferred, but not required.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal skills and ability to communicate effectively with clients, families, and healthcare teams.
