About the Role:

As an LPN at Sunrise Senior Living, you will play a vital role in providing high-quality care to our residents. Your primary responsibilities will include:

  • Conducting monthly wellness visits for all residents
  • Monitoring and following up with residents who have experienced any falls, accidents, or incidents
  • Documenting all pertinent information in the resident wellness file
  • Maintaining communications with residents' families and responsible parties to ensure medical needs are being met
  • Providing clinical support and assistance to community team members as needed
Requirements:

To be successful in this role, you will need:

  • A graduate degree in nursing from an approved college or school
  • A current state license as a professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN)
  • Minimum of one (1) year experience in home health, assisted living, hospital, or long-term care environment
  • Knowledge of nursing practices, techniques, and methods applied to health and wellness resident care, and federal, state, and local regulations
