Position Overview
The PA or NP will possess a deep commitment to delivering high-quality primary and preventive care, focusing on continuity of care, building enduring relationships with patients, and promoting health and wellness strategies while managing complex chronic conditions. Key responsibilities include:
- Deliver comprehensive outpatient primary care services for both adults and children.
- Develop diagnoses based on patient history and physical assessments.
- Initiate, review, and adjust treatment plans to ensure optimal patient well-being.
- Collaborate effectively with a multidisciplinary team, including one physician and four health coaches dedicated to supporting patient wellness goals.
- Coordinate and expand a network of reliable external medical consultants, hospitalists, emergency room providers, and support services.
- Lead or assist in community-based initiatives aimed at enhancing patient engagement, access, and clinical quality performance.
- Consult with the supervising physician on cases as outlined in the delegation agreement.
- Travel is required to various clinics and community sites.
- Undertake additional responsibilities as assigned.
Required Qualifications
- Valid California Physician Assistant (PA) license or California Nurse Practitioner (NP) license.
- Current DEA registration is mandatory.
- Proficient in technology, including MS Office, tablets, smartphones, and EMR systems.
- Bilingual proficiency in Spanish is strongly preferred, with Medical Spanish being essential.

Additional Benefits
- Compensation: $150,000 - $175,000, with eligibility for a discretionary bonus. Final salary will be based on skills and experience. This role also includes a monthly car allowance.
