Hospice Care Nurse Specialist

Amedisys seeks a skilled Hospice Care Nurse Specialist to provide continuity and crisis care services in patient homes. This role involves collaborating with healthcare professionals to deliver high-quality patient care.
About the Role:This position is ideal for a licensed practical nurse (LPN) looking for a challenging and rewarding career opportunity. With Amedisys, you'll have the flexibility to work on a PRN basis, with varied hours from 5pm to 8am.
Responsibilities:- Verify the patient's plan of care with the registered nurse prior to each visit.
- Collaborate frequently with physicians and registered nurses to ensure patient needs are met.
- Teach patients and caregivers self-care techniques to promote independence and well-being.
- Monitor vital signs and review medications at each visit.
- Perform specialized procedures ordered by the physician.
- Prepare equipment and materials for treatments using aseptic technique.
- Complete timely and accurate documentation.

Requirements:- 1+ year(s) of clinical LPN experience.
- Current LPN license, specific to the state(s) assigned.
- Current CPR certification.
- HCBH and/or hospice experience highly preferred.
- Valid driver's license, reliable transportation, and liability insurance.
