Job Summary

VNA Health Group is seeking a skilled Hospice Nurse Liaison to join our dedicated team in Middlesex County. The successful candidate will be responsible for assessing patients for hospice needs and coordinating total care for hospice patients and their families.

Key Responsibilities
  • Assess patients for hospice needs and provide counseling to patients and their families
  • Coordinate the transition to hospice to promote continuity of care
  • Establish and maintain liaison with physicians, external and internal customers
  • Assist in managing and monitoring the quality and appropriateness of services and programs
  • Conduct community outreach activities to improve community access to hospice care
Requirements
  • Bachelor's degree in Nursing required
  • Excellent clinical skills and experience in hospice nursing and community health nursing
  • Excellent verbal and written communication skills
  • Excellent customer service, organizational and interpersonal skills
  • Strong computer skills with proficiency in MS Office Suite
