Position OverviewGeneral Summary
With limited supervision, this role involves delivering a high level of comprehensive healthcare to a designated patient demographic in partnership with medical personnel. Adhering to established nursing and medical protocols, the individual will provide specific care instructions to nursing and support staff.
Key Responsibilities
The following responsibilities outline the general nature and scope of work for this position. This list is not exhaustive and may include additional duties as assigned.
- Conducts thorough medical assessments, diagnoses, treatments, follow-ups, consultations, and health education tailored to the patient's age and circumstances.
- Collects patient histories and develops care plans, ensuring thoroughness and accuracy in documentation.
- Orders and interprets necessary diagnostic tests.
- Creates, orders, and documents suitable treatment plans, including medication prescriptions based on a detailed review of health history and diagnostic findings.
- Communicates care plans effectively with relevant healthcare providers, including collaborating physicians, primary care providers, nursing staff, and case managers.
- Engages with patients and their families to explain care plans, disease processes, new treatment options, and medication schedules.
- Delivers medical care to patients experiencing illness or injury, adhering to accepted standards of practice.
- Provides health education to patients and families as necessary.
- Records medical findings and care provided in the patient's clinical documentation, including laboratory and imaging results.
- Educates patients on health maintenance and provides necessary resources.
- Collaborates with physicians and healthcare team members regarding patient health needs, outcomes, and the effectiveness of care plans.
- Initiates contact with other healthcare providers as needed.
- Ensures the confidentiality and security of all protected health information, disclosing only what is necessary for job functions.
Education and Experience
- Master's degree with completion of an accredited Nurse Practitioner Program is required.
- Current state licensure as a Nurse Practitioner is required.
- Current state licensure as a Registered Nurse is required.
- Current BLS certification or obtained prior to delivering direct patient care is required.
- Experience is preferred but not mandatory.
- Eligibility for DEA and CDS is required.
Knowledge, Skills, and Abilities
- Demonstrates knowledge and skills necessary to provide care appropriate to the patient populations served, including understanding growth and development principles across the lifespan.
- Exhibits strong verbal and written communication skills for effective interaction with patients, families, and healthcare teams. Proven interpersonal skills are essential.
- Maintains up-to-date hospital safety and mandatory training.
- Holds current immunizations as recommended for the practice area.
Patient Safety Commitment
Ensures patient safety in all job functions and actively participates in patient safety initiatives within the hospital or department.
- Takes proactive measures to address any identified risks to patient safety.
- Reports adverse events and near misses to the appropriate management authorities.
- Identifies potential risks in processes, procedures, and devices, communicating these to responsible parties.
