Job Title: Home Health Nurse - LPN

We are seeking a skilled and compassionate Home Health Nurse - LPN to join our team of professionals. As a Home Health Nurse - LPN, you will be responsible for providing high-quality patient care in the comfort of their own homes.

Responsibilities:
  • Assess patients' condition and initiate start of care face-to-face visits
  • Perform evaluation tasks including vital signs and medication review
  • Administer medication as prescribed by the patient's physician
  • Dress (or re-dress) wounds
  • Provide patient education
  • Coordinate with the interdisciplinary team regarding care plan progress
  • Make recommendations for devices/tools/medications that might improve patient's quality of life
Requirements:
  • Must be 18 years or older
  • Must have successfully completed a FL-approved LPN program from an industry-accredited institution
  • Must have reliable transportation and proof of auto insurance
  • Must successfully complete a Level II background check prior to offer and initiation of training
  • Must complete a competency check and all orientation prior to initiating patient care
  • Current FL State LPN license is required
Work Environment:

We serve the following counties: Lake, Sumter, Citrus, and Hernando. As a Home Health Nurse - LPN, you will have the opportunity to work with a diverse range of patients and families in a dynamic and supportive environment.

Job Type:

This is a per diem, contract (1099) position. You will be paid per visit, with rates varying based on experience.
