We are seeking a highly skilled and experienced Pharmacist Sr to join our Ambulatory Care team at UCI Health. As a key member of our clinical team, you will be responsible for providing exceptional patient care and contributing to the development of our ambulatory care services.
Key Responsibilities- Provide clinical pharmacy services in ambulatory care settings, including medication therapy management, patient education, and clinical monitoring.
- Collaborate with healthcare teams to develop and implement patient-centered care plans.
- Conduct medication use evaluations and provide recommendations for improvement.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.
- Precept pharmacy residents, students, and technicians as needed.
- 3+ years of clinical experience as a licensed pharmacist in an ambulatory care setting.
- Possession of the legal requirements for the practice of pharmacy as determined by the CA Board of Pharmacy.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- Current BLS or ACLS certification.
- Pharm.D. degree.
- Hospital pharmacy residency.
