Job SummaryWe are seeking a qualified Registered Nurse or Licensed Practical Nurse to join our team in a part-time capacity at our Perrysburg, OH office. The ideal candidate will possess an active RN or LPN license issued by the state of Ohio and demonstrate excellent interpersonal and customer service skills.
Responsibilities- Monitor the physical health of individuals served, providing nursing services and witnessing urine screens.
- Communicate with healthcare practitioners regarding client health status and provide consultation to staff regarding health care issues.
- Collaborate with the team to ensure comprehensive health histories are obtained and referrals are made as needed.
- Provide education to individuals served, family members, and staff members regarding medication, symptom management, wellness, and health.
- Assess individuals' mental illness or substance abuse and physical symptoms, communicating changes to the treatment team and making adjustments to treatment plans.
- Active RN or LPN license issued by the state of Ohio.
- Two years of nursing experience.
- Associate's or Bachelor's Degree in nursing or a medical-related field from an accredited college or university.
- Experience in a psychiatric or substance use treatment setting.
