Job Title: Licensed Practical Nurse

At Sunrise Senior Living, we are committed to providing high-quality care to our residents. As a Licensed Practical Nurse, you will play a vital role in ensuring the overall well-being of our residents.

Responsibilities:
  • Conduct regular wellness visits for all residents to identify their ongoing needs and services.
  • Collaborate with the Resident Care Director to evaluate and document changes in resident health care services and status.
  • Assist in conducting resident evaluations and gathering data at move-in, 30 days post-move-in, and semi-annually.
  • Monitor and follow up with residents who have experienced falls, accidents, or incidents, documenting and reporting as required.
  • Document all pertinent information in the resident wellness file.
  • Maintain open communication with residents' families and responsible parties to ensure their medical needs are being met.
  • Communicate with residents' attending physicians as necessary and upon family request.
  • Ensure weights and vital signs are obtained monthly for each resident.
  • Assist in maintaining wellness files according to Sunrise policies and federal and state regulations.
  • Provide clinical support and assistance to community team members as needed.
  • Understand and follow infection control practices.
  • Partner with the RCD and community team to promote infection control standards, including resident testing, training, and skills checks.
  • Regularly monitor each resident's medication and treatment profile to ensure accurate administration and documentation.
Qualifications:
  • Graduate of an approved college or school of nursing.
  • Current state license as a professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN).
  • Minimum of one year of experience in home health, assisted living, hospital, or long-term care environment.
  • Demonstrated knowledge of nursing practices, techniques, and methods applied to health and wellness resident care, and federal, state, and local regulations.
  • Ability to handle multiple priorities and possess written and verbal skills for effective communication.
  • Competent in organizational, time management skills, and good judgment, problem-solving, and decision-making skills.
  • Proficient in computer skills, Microsoft Office (Windows, Outlook, Excel), and Sunrise applications with the ability to learn new applications.
