Job Summary:

We are seeking a skilled Bereavement Support Coordinator to join our team at Amorem. This role involves providing emotional support and counseling to individuals who have experienced loss, as well as educating community members about bereavement services.

About the Role:

The Bereavement Support Coordinator will work closely with patients, families, and community groups to provide individualized support and education. This may include conducting home visits, facilitating support groups, and presenting educational programs in local schools.

Key Responsibilities:

  • Assess the bereavement needs of assigned individuals on an ongoing basis
  • Develop and implement personalized plans of care to address these needs
  • Promote and coordinate presentations in the local school system to educate students and staff about bereavement services
  • Serve as a resource to community individuals and groups about bereavement services
  • Collaborate with school personnel to provide grief education and support to students
  • Network with community school counselors and social workers to promote bereavement services
  • Provide support and education to children and families served by Amorem
  • Facilitate bereavement support groups and presentations as needed

Requirements:

  • Master's degree in social work, psychology, or human service degree
  • Two years' professional experience required
  • Experience in bereavement preferred
  • Knowledge of and commitment to the hospice philosophy of care
  • Ability to work on an interdisciplinary team
  • Ability to work a flexible schedule
