About the Role:
The Clinical Care Coordinator is responsible for the seamless delivery and coordination of patient care, utilizing the Nursing Process in alignment with organizational standards and policies. This role plays a crucial part in the success of the Patient-Centered Medical Home model by collaborating with patients, healthcare providers, and insurance plans to guide and direct patient care effectively.
Key Responsibilities:
- Deliver daily care coordination and nursing services to patients with chronic conditions, advocating for evidence-based practices and quality improvement initiatives.
- Assist physicians with patient evaluations, examinations, and medical procedures.
- Conduct thorough pre-visit assessments and document patient care activities in accordance with professional standards.
- Develop and maintain comprehensive patient care plans, ensuring compliance with NCQA PCMH and other accreditation requirements.
- Provide education and support to patients and their families, helping them navigate their health challenges.
Qualifications:
- Graduate from an accredited School of Nursing.
- Current Registered Nurse license in the State of Ohio.
- Minimum of two years of clinical nursing experience in an inpatient hospital setting.
- Preferred experience in an ambulatory care environment and case management.
