We are seeking a highly skilled Registered Nurse to join our Case Management team at INTEGRIS Health. As a RN Case Manager, you will play a critical role in ensuring the seamless transition of patients through the healthcare system, promoting optimal clinical and financial outcomes.
Key Responsibilities- Conduct comprehensive assessments of patients' clinical, psychological, and financial needs, utilizing all available resources.
- Develop, implement, and evaluate plans for discharge, including referrals to other healthcare and community organizations as needed.
- Communicate discharge care plans and any changes to patients, families, and healthcare professionals.
- Collaborate with physicians and hospital staff to ensure timely and efficient patient discharge.
- Coordinate services between hospital departments to facilitate timely patient discharge.
- Conduct concurrent review of patient records to ensure accurate utilization of resources.
- Apply utilization criteria accurately to determine appropriate resource utilization.
- Notify internal and external contacts of utilization issues that may affect patient care and/or reimbursement.
- 5 years of experience in various clinical settings, including home health, inpatient, physician office, and clinic.
- Experience with managed care and payer/provider requirements preferred.
- Current licensure as a Registered Nurse in the State of Oklahoma or current multistate license from a Nurse Licensure Compact (eNLC) member state.
- BSN preferred.
- Excellent interpersonal, communication, and collaboration skills.
- Computer experience required, with Windows preferred.
- Current Case Management Certification preferred, with a minimum of one year of experience.
