At TN Health Care, we are seeking a skilled and compassionate Home Health Aide to join our team. As a Home Health Aide, you will play a vital role in enabling patients to stay in their homes by providing support and personal services, teaching families, and monitoring patient condition.
Key Responsibilities:
- Monitor patient condition by observing physical and mental condition, intake and output, and exercise.
- Provide housekeeping and laundry services, shopping for food and other household requirements, preparing and serving meals and snacks, and running errands.
- Assist patients with personal services, such as bathing, dressing, and grooming.
- Teach patients and their families how to use special equipment and techniques for personal hygiene.
- Help patients care for themselves by teaching the use of canes or walkers, special utensils to eat, and other techniques.
- Advise on nutrition, cleanliness, and housekeeping.
- Record patient information by making entries in the patient journal and notifying the nursing supervisor of changing or unusual conditions.
- Maintain a safe, secure, and healthy patient environment by following asepsis standards and procedures.
Requirements:
- High School Diploma or GED
- CPR/FA, 1 & 2 (within 6-12 months of hire), and CPI certification
- Any state-mandated certification
- Minimum 1+ year(s) professional or personal experience supporting individuals with disabilities, mental illnesses, or challenging behaviors
- Valid in-state driver's license, current registration, and insurance, with a driving record of no more than 2 points within the last 3 years
- Must be over 21 years old
