Salary:
The hourly range for this position is $41.22 - $63.59, dependent upon experience.

Job Description:
This role specializes in speech and feeding interventions, helping patients improve both their communication and feeding skills. The position entails thorough documentation, compliance with care standards, and active participation in patient care coordination.

Required Skills and Qualifications:
To succeed in this role, you will need:
  • A valid/current Utah Speech Language Pathology License.
  • A current driver's license, insured, reliable transportation, and an acceptable driving record.
  • Basic Life Support Certification (BLS) for healthcare providers.
  • A current Certificate of Clinical Competence.
  • Effective communication skills.
  • Basic computer skills.

Job Essentials:
  1. Provides timely and thorough documentation of the patient record consistent with agency standards.
  2. Practices within the Scope of Practice and complies with all policies, laws, licensure, regulations, and standards for care pertaining to the delivery of home care services.
  3. Demonstrates competent theoretical and technical skills pertinent to patient populations being served: Dysphagia, Speech/Language Pathology, Communication skills, Cognition, Voice, Feeding skills.
  4. Demonstrates compliance with patient care coordination: weekly and monthly chart audits as requested, utilization, infection control, improvement in corrective action plans, and attendance at staff and training meetings.
  5. When assigned, demonstrates effective primary patient care coordination with all disciplines.
  6. Follows the patient care process: thoroughly identifies and evaluates functional and rehabilitative needs, identifies and prioritizes patient needs, develops and implements appropriate care plan and goals, adjusts and updates care plan as needed, appropriately coordinates patient care with other disciplines, provides appropriate education to patients and families, consistently follows the established care plan.
  7. Demonstrates the efficient use of resources by effectively managing time, supplies, and equipment and by meeting established productivity standards.
