**Job Summary:**

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team at Montgomery Nursing and Rehabilitation Center. As a LPN, you will play a vital role in providing high-quality care to our residents, working collaboratively with the care team to deliver exceptional patient outcomes.

**Responsibilities:**

* Provide daily support to the unit leadership and respond timely to resident and family needs within the LPN scope of practice
* Use nursing process, judgment, and skills to set priorities necessary to deliver quality direct resident care and make decisions promoting the safety and quality of care
* Perform and administer standard nursing treatments and/or medications and observe residents for desired or unwanted effects
* Communicate and collaborate with the care team to revise and update the Nursing Care Plan or any pertinent findings, including lab and test results, incidents, and/or concerns
* Monitor and document vitals, treatments administered, and residents' responses
* Prepare, organize, oversee, monitor, and evaluate the delivery of care of resident CNA assignments and activities, considering the category of needs and competency of personnel

**Requirements:**

* Graduate of an approved school of nursing
* Licensed and currently registered in good standing in New York as an LPN
* Dedicated to providing extraordinary, clinically sound, and compassionate care
* Must possess a spirit of cooperation, caring, and enthusiasm to create an atmosphere conducive to rehabilitation and optimal resident health
* Genuinely interested in helping ill, injured, disabled, elderly, emotionally upset, and sometimes challenging residents
* Have excellent communication, customer service, and organization skills
* Work well under pressure and willing to work harmoniously with all staff
