Job Summary

We are seeking a skilled Licensed Practical Nurse to join our interdisciplinary team at Bridgeview Estates. As a key member of our healthcare team, you will provide high-quality nursing care to patients, promoting their physical, mental, and psychosocial well-being.

Responsibilities
  • Deliver quality nursing care to patients through interpersonal contact and provide care and services to allow patients to attain or maintain the highest practicable physical, mental, and psychosocial well-being.
  • Monitor and report changes in the patient's condition, ensuring timely interventions and adjustments to care plans as needed.
  • Assign duties, instruct, and provide clinical supervision to Certified Nursing Assistants (CNAs), promoting a culture of teamwork and collaboration.
  • Maintain accurate and detailed records of patient care, including progress notes, medication administration, and vital signs.
  • Collaborate with interdisciplinary teams, including physicians, therapists, and other healthcare professionals, to develop and implement comprehensive care plans.
  • Participate in quality improvement initiatives, contributing to the development and implementation of policies and procedures that enhance patient care and safety.
Requirements
  • Graduation from an accredited Licensed Practical Nurse (LPN) program.
  • Current licensure/registration in applicable State, with a valid LPN/LVN license in good standing throughout employment.
  • One (1) year of skilled nursing experience, preferably in a long-term care or rehabilitation setting.
  • CPR certification upon hire or obtain during orientation, with ongoing maintenance of current certification.
