As a Registered Nurse (RN) Home Health Case Manager, you will play a vital role in creating, implementing, and executing comprehensive plans of care to promote wellness and prevent hospitalization.
You will be responsible for overseeing wound care, blood draws, tube feedings, Tracheostomy care, foley catheters, Congestive Heart Failure (CHF) management, and medication education in patients' homes.
Your responsibilities may include:
- Conducting full assessments of patients and families during initial visits.
- Developing advanced plans of care, including rehabilitation, based on assessment data.
- Working one-on-one with patients and families to achieve improvement or stability.
- Managing patients' care episodes in collaboration with interdisciplinary clinical teams.
- Preparing clinical notes, orders, and charting within compliant timeframes.
- Collaborating with physicians and agency staff to deliver exceptional care.
The ideal candidate will possess:
- A current and unrestricted California RN license.
- Current BLS/CPR certification.
- A valid California driver's license.
- High-level clinical knowledge in home health care, preferably with experience working with high-acuity patients.
- Experience with case management fundamentals and working within interdisciplinary teams.
- Self-directed, empathetic, critical thinking, and strong interpersonal skills.
- Leadership abilities and organizational capacity.
Bonus incentives and benefits include:
- An estimated annual salary of $120,000-$135,000, plus a quarterly bonus of up to $10,000.

The successful candidate must be willing to work either Saturday or Sunday, depending on operational needs. The company prioritizes accommodating assigned primary territories; however, temporary reassignments may occur to support safe patient care and meet growing business demands.
