Key Responsibilities
  • Provide high-quality patient care in the comfort of their own homes, as prescribed by the physician and in compliance with applicable laws and regulations.
  • Coordinate with other healthcare professionals to ensure seamless care and maintain continuity of patient care.
  • Conduct comprehensive assessments and develop individualized plans of care to meet the unique needs of each patient.
  • Ensure patients meet home health eligibility and medical necessity guidelines as defined by payer sources.
Requirements
  • Current RN licensure in the state of practice.
  • Current CPR certification.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
