Job Summary:
We are seeking a skilled Physical Therapist to join our team at HexaQuEST Health, Inc. The ideal candidate will have experience in outpatient settings and be able to work effectively with patients to achieve their rehabilitation goals.

Responsibilities:
• Provide high-quality physical therapy services to patients
• Conduct thorough assessments and develop individualized treatment plans
• Collaborate with other healthcare professionals to ensure comprehensive care
• Maintain accurate records and documentation
• Meet productivity expectations and maintain a high level of patient satisfaction

Requirements:
• 3 references required for candidate to start
• Outpatient experience preferred
• Will accept new graduates
• Will not accept first-time travelers
• 36 hours guaranteed with the expectation of working up to 40 hours
• OT must be approved by director before any OT hours are worked

Work Environment:
• Float to other clinics: No

Patient Population:
• Productivity expectation is 60 visits/week
• Will see 12-14 patients/day
• Double-booking of follow-up visits expected
