Key Responsibilities
  • Assessment and Treatment Planning

Conduct thorough assessments of patients' physical abilities and develop personalized treatment plans to help them achieve their goals.

Delivery of Physical Therapy Services

Provide one-on-one physical therapy sessions to patients in their homes, using a variety of techniques and modalities to promote healing and improvement.

Documentation and Communication

Maintain accurate and detailed records of patient progress and communicate effectively with patients, families, and healthcare teams to ensure seamless care.

Collaboration and Teamwork

Work collaboratively with other healthcare professionals to ensure comprehensive care and support for patients and their families.

Requirements
  • Education and Licensure

Master's degree in Physical Therapy from an accredited program and current licensure as a Physical Therapist in the state of practice.

Experience and Skills

At least 1 year of experience as a Physical Therapist, with a strong background in home health care and a proven ability to work independently and as part of a team.

Preferred Qualifications

Experience with OASIS documentation and a strong understanding of home health care regulations and guidelines.
