Job Summary

As a RN Case Manager at Trinity Health, you will play a vital role in implementing an interdisciplinary process for evaluating the options and services required to meet an individual's healthcare needs. This includes conducting initial and concurrent chart reviews for utilization review and identifying patients requiring case management follow-up.

Responsibilities

• Conduct thorough assessments of patients' healthcare needs and develop individualized plans to meet those needs.
• Collaborate with healthcare teams to ensure seamless transitions of care and optimal patient outcomes.
• Conduct chart reviews to identify opportunities for improvement and implement changes as needed.
• Develop and maintain relationships with patients, families, and healthcare providers to ensure effective communication and coordination of care.

Requirements

• Current GA RN license.
• Graduate of an accredited school of nursing, BSN recommended.
• Minimum of 2 years' experience in nursing and 1 year in case management, utilization management, or discharge planning preferred.
• Certification in case management preferred.
