Custom Healthcare Solutions is seeking a skilled Home Health RN to join our team as a local contract nurse. This is a fantastic opportunity for a dedicated and compassionate nurse to provide high-quality patient care in a home health setting.
Key Responsibilities:
- Provide comprehensive home health care services to patients in Burlington County, New Jersey.
- Assess patient needs and develop individualized care plans to ensure optimal health outcomes.
- Collaborate with healthcare teams to coordinate patient care and services.
- Conduct regular patient visits and document care provided.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions.
Requirements:
- Current RN licensure in the state of New Jersey.
- Recent experience in acute care or home health nursing.
- Liability insurance and car insurance.
- Ability to work independently and as part of a team.
- Excellent communication and interpersonal skills.
