Key Responsibilities
  • Conduct comprehensive assessments of patients' bio-psycho-social needs and develop individualized care plans.

Requirements
  • Associate's Degree in Nursing required; Bachelor's Degree in Nursing preferred.
  • 1 year of nursing experience preferred, with prior home health, clinical, and direct patient care experience.
  • Ability to work independently and effectively communicate with patients and healthcare teams.
