We are seeking a highly skilled and compassionate Social Worker/Case Manager to join our team at Family Service Rochester. As a key member of our staff, you will play a vital role in providing supportive services to pregnant and parenting individuals who have a history of substance use.
Key Responsibilities- Conduct Comprehensive Assessments: Conduct thorough assessments to identify strengths and barriers that affect family stability, including employment, housing, healthcare, and social/emotional challenges that impact child safety and well-being.
- Develop Individualized Case Plans: Create personalized case plans to guide and support clients in achieving their goals in areas such as sobriety, prenatal and postnatal care, parenting skills, and child development.
- Coordinate Services: Collaborate with community service providers, including Child Protection, Public Health, Community Corrections, Mental Health/Chemical Dependency treatment providers, and other relevant agencies to ensure comprehensive support for families.
- Engage in Outreach Activities: Build and maintain a network of resources, advocate for families, and engage in outreach activities to support clients in achieving their goals.
- Facilitate Support Groups: Co-facilitate weekly or bi-weekly sober support and education groups, as well as coordinate and host parent-child activities as scheduled.
- Document and Report: Complete all required documentation in a timely manner, including time reporting, case notes, and family support plans.
- Education and Experience: Bachelor's degree in a human services-related field required; preference given to those with experience in substance use, child welfare, direct work with diverse populations, and bilingual skills.
- Licensure: Preference given to those with a Minnesota Social Work License or license eligibility.
