Patient Care Coordinator

About the RoleThis is an exciting opportunity to work in a dynamic and supportive environment, where you will be responsible for coordinating patient care and ensuring the delivery of exceptional service. As a Patient Care Coordinator, you will be working closely with a multidisciplinary team to provide comprehensive care to patients with complex needs.
The ideal candidate will have strong communication and organizational skills, with the ability to work effectively in a fast-paced environment. You will be expected to maintain accurate records, coordinate patient transfers, and communicate with patients, families, and healthcare teams to ensure seamless care delivery.
Key Responsibilities- Coordinate patient care activities, including admissions, discharges, and transfers
- Maintain accurate and up-to-date patient records
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care delivery
- Collaborate with multidisciplinary teams to develop and implement patient care plans
To be successful in this role, you will need:
- A degree in Nursing (A.D.N or B.S.N) from an accredited institution
- Experience working in a healthcare setting, preferably in dialysis or nephrology
- Strong communication and organizational skills
- Ability to work effectively in a fast-paced environment
As a valued member of our team, you can expect a competitive salary range of $70,000 - $100,000 per year, depending on experience. In addition, we offer a comprehensive benefits package, including medical, dental, and vision insurance, 401(k) matching, and paid time off.
