We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Independence Plus. As a LPN, you will provide high-quality, patient-centered care to medically complex adult and pediatric patients in their homes. Our team is dedicated to supporting our nurses in their career advancement and professional growth.

Responsibilities:
  • Provide one-on-one home healthcare to patients with complex medical and respiratory needs
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Collaborate with our interdisciplinary team to ensure seamless care coordination
  • Participate in ongoing education and training to enhance your skills and knowledge
Requirements:
  • Current IL Licensed Practical Nurse (LPN) license
  • 1 year of clinical experience preferred for field nurses
  • Current CPR certification (AHA Healthcare Provider)
