Job Description:

We are seeking a Transitions Care Coordinator to join our team. The successful candidate will be responsible for facilitating a seamless transition for patients from a facility to their home environment.


Responsibilities:

  • Assess and plan patient care needs
  • Coordinate with healthcare providers to ensure continuity of care
  • Develop and implement plans of care
  • Monitor and evaluate patient progress
  • Maintain accurate records and documentation

Requirements:

  • CURRENT LPN or RN license in the state of service
  • At least one year of experience in home health or hospital case management
  • Reliable means of transportation and current driver's license and auto insurance

Preferred Qualifications:

  • Experience in Home Health or healthcare business development
