About the Role- Assess and evaluate patients for physical therapy needs and develop personalized plans of action in collaboration with physicians.
- Provide skilled physical therapy care to patients in their homes, promoting independence and improving functional abilities.
- Document all changes in patient condition and update care plans accordingly.
- Identify counseling needs and plan appropriate teaching strategies to support patients and their families.
- Monitor quality and appropriateness of services provided and adhere to agency infection control and safety policies.
- Physical Therapy Licensure in NJ required.
- 2-3 years of prior clinical experience and community-based experience preferred.
- Strong computer skills and effective communication and interpersonal skills.
- Valid NJ driver's license.
