About the Job:

  • Develop and implement individualized care plans for clients with diverse needs and circumstances.
  • Collaborate with internal and external stakeholders to ensure seamless service delivery and effective communication.
  • Provide education and guidance on HIV/AIDS-related topics, promoting health literacy and empowerment among clients.
  • Ensure timely and accurate documentation of client interactions and service delivery.
  • Maintain confidentiality and adhere to HIPAA guidelines in all interactions with clients and colleagues.

Requirements:

  • Bachelor's degree in Social Work or related field, or equivalent experience.
  • 2+ years of experience in case management, social services, or healthcare.
  • Familiarity with HIV/AIDS protocols and treatment options.
  • Excellent communication and interpersonal skills.
  • Able to work independently and as part of a team.
