We are seeking a highly skilled Clinical Pharmacist to join our team at Lee Health. As a Clinical Pharmacist, you will play a critical role in improving patient outcomes through evidence-based prescribing practices and medication management.
Key Responsibilities- Conduct medication use evaluations and support patient access and adherence to therapy
- Utilize analytic data to determine which patients will benefit from interventions, medication education, and collaboration with primary care providers and specialists
- Perform detailed medication reconciliation and provide recommendations to providers regarding utilization of high-cost medications
- Participate in the development of quality measures and quality improvement initiatives
- Provide educational presentations to providers and technician supervision to ensure accurate and timely submissions of prior authorizations
- Doctor of Pharmacy degree or BS Pharmacy degree with equivalent and acceptable experience from a College of Pharmacy accredited by the American Association of Colleges of Pharmacy
- At least 3 years of inpatient or ambulatory pharmacy experience or completion of PGY 1
- Certification in Ambulatory Care Pharmacy Specialty (BCACP) and Collaborative Practice Agreement certificate
- Strong communication and interpersonal skills, analytical skills, and leadership skills
- Proficient in the use of electronic medical records (EMR)
- Willingness to serve as a preceptor to pharmacy students and residents
