CenterWell Senior Primary Care is seeking a highly skilled and compassionate Primary Care Physician to join our team. As a Senior Medical Professional, you will play a vital role in providing proactive, preventive care to seniors, including wellness visits, physical exams, chronic condition management, screenings, and minor injury treatment.
Key Responsibilities:
- Provide personalized care to patients, taking the time to listen, learn, and address the factors that impact their well-being.
- Work collaboratively with our integrated care teams, including physicians, nurses, behavioral health specialists, and more, to deliver compassionate and effective care.
- Develop and implement treatment plans that address the physical, emotional, and social needs of patients.
- Stay up-to-date with the latest medical research and best practices to ensure the highest quality care for our patients.
- Participate in quality improvement initiatives to continuously improve patient outcomes and experiences.
Requirements:
- Active and unrestricted state license to practice medicine.
- Board certification in a relevant medical specialty.
- Minimum 2 years of clinical experience in primary care or a related field.
- Strong technical skills, including proficiency in electronic medical records (EMRs) and other healthcare software.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
