About the Role:

We are seeking a compassionate and skilled Home Health Aide to provide personal care services to patients in their homes. As a Home Health Aide, you will work closely with our team to deliver high-quality care and support to individuals with chronic and acute illnesses and disabilities.

Responsibilities:

  • Assist patients with daily living activities, such as bathing, dressing, and toileting, as per their individual care plans.
  • Take and record vital signs, such as temperature, pulse, and blood pressure, as ordered by healthcare professionals.
  • Provide safe transfer and ambulation techniques to patients, ensuring their safety and well-being.
  • Perform range of motion exercises and other simple procedures as an extension of therapy services, as ordered by healthcare professionals.
  • Meet safety needs of patients and use equipment safely and properly.
  • Observe and teach home safety practices to patients and their families.
  • Recognize emergency situations and know emergency management procedures.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care delivery.

Requirements:

  • Meet all training requirements in accordance with state and federal laws.
  • Ability to read, follow written instructions, and document care given.
  • Possess and maintain current CPR certification.
  • PPD or chest x-ray if applicable.
  • Successful completion of Maxim screening examinations.
  • Participation and completion of Maxim's competency program.
