Job Summary: The Clinical Case Manager-ED plays a critical role in the patient care delivery team at Loma Linda University Medical Center (LLUMC). This role involves utilizing clinical expertise to determine medical necessity and appropriate patient care in the Emergency Department. The successful candidate will possess a Bachelor's Degree in Nursing or an Associate's Degree in Nursing with six years of case management experience, as well as a minimum of three years of bedside nursing experience in a hospital setting. Additionally, one year of case management experience is preferred.
Key Responsibilities:
• Determine medical necessity and develop individualized care plans for patients in the Emergency Department
• Collaborate with healthcare teams to ensure seamless transitions of care
• Utilize clinical expertise to identify opportunities for improving patient outcomes and reducing length of stay
• Develop and maintain relationships with community resources and organizations to support patient needs
