Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team as a Hospice LPN. As a Hospice LPN, you will provide direct patient care services, evaluate patient needs, and assist in the preparation and execution of the plan of care. You will work closely with our interdisciplinary team to ensure that patients receive the highest level of care and support.

Responsibilities
  • Provide direct patient care services, including administering medications, wound care, and other treatments as needed.
  • Evaluate patient needs and assist in the preparation and execution of the plan of care.
  • Work closely with the interdisciplinary team to ensure that patients receive the highest level of care and support.
  • Document patient information and communicate with the healthcare team as needed.
Requirements
  • Licensed Practical Nurse (LPN) license to practice in the state of employment.
  • 1+ years of patient care experience in hospice, home health, ICU, geriatric, or oncology.
  • Current driver's license and automobile insurance.
  • Current CPR certification.
