Soliant Health is seeking a highly skilled Oncology Pharmacist Specialist to join our team in Colorado. As a key member of our healthcare team, you will play a critical role in providing exceptional patient care and support.
Key Responsibilities- Medication Management: Safely oversee the compounding and dispensing of chemotherapy and other non-hazardous infusions, ensuring accurate medication lists and minimizing drug waste.
- Collaboration and Communication: Work closely with pharmacy technicians, healthcare providers, and patients to develop and implement effective treatment plans, provide initial teaching for patients starting new treatment regimens, and serve as a USP advisor.
- Education and Training: Provide educational in-services to pharmacy and other hospital staff, as well as community health professionals and consumers, to promote evidence-based practices and improve patient outcomes.
- Leadership and Representation: Represent pharmacy at hospital or medical staff committees, as assigned by the Director, and oversee patients enrolled in REMS programs, assist with prior authorizations, and provide patient education.
- Program Development: Implement and maintain oral chemotherapy follow-up programs, ensuring seamless patient care and support.
- Education: Pharmacy degree (BS or Pharm.D.) from an ACPE-approved college of pharmacy.
- Licensure: Licensed, or eligible for licensure, pharmacist in Colorado.
- Certification: Board certification in oncology desirable.
- Skills: Residency or fellowship training in oncology desirable, minimum 2 years oncology experience required.
