We are seeking a skilled Physical Therapist Care Manager to join our team at AdventHealth Home Care West FL. As a Care Manager, you will be responsible for coordinating and directing the care of a caseload of home patients requiring physical therapy as the primary skill.

Key Responsibilities:

  • Coordinates and directs the care of a caseload of home patients requiring physical therapy as the primary skill.
  • Performs comprehensive assessments, develops and implements individualized care plans, and evaluates patient progress.
  • Collaborates with patients, families, caregivers, and healthcare providers to ensure seamless care coordination.
  • Identifies performance improvement opportunities and implements initiatives to enhance patient outcomes and care delivery.

Requirements:

  • Current Physical Therapy License in State of Practice
  • Valid Driver's License and current car insurance
  • BLS certified
  • Minimum of one-year relevant clinical physical therapy experience
  • Master's Degree in Physical Therapy

Preferred Qualifications:

  • Home Health Case-Manager Certification
  • COS-C Certification
  • A working knowledge of community resources and an ability to refer patients and families appropriately
