We are seeking a highly skilled Oncology Nurse Navigator to join our dynamic team at Ashe Memorial Hospital. As a key member of our oncology department, you will play a vital role in providing exceptional patient care and navigation services to individuals with cancer.
Key Responsibilities- Assess and identify high-risk patients, providing physical and psychological support to patients, families, and caregivers.
- Collaborate with physicians and multidisciplinary teams to develop and implement individualized care plans.
- Provide education and support to patients and families, addressing their needs and concerns throughout the cancer journey.
- Facilitate timely scheduling of appointments, diagnostic testing, and procedures to ensure continuity of care.
- Work closely with patients and families to ensure discharge instructions are understood and followed.
- BSN required.
- 5 years of previous oncology nursing experience preferred.
- Knowledge of clinical practice and oncology care.
- Must be licensed as a Registered Nurse (RN) in the state of NC.
- Current BLS Certification.
- ACLS certification obtained within 6 months of hire.
- Oncology Certified Nurse Preferred.
- ONS Chemotherapy/Biotherapy certification required.
