About the Role

We are seeking a skilled and compassionate Home Health Registered Nurse to join our team at Interim HealthCare. As a Home Health Registered Nurse, you will provide high-quality, patient-centered care to individuals in the comfort of their own homes.

Key Responsibilities
  • Provide comprehensive home-based care to patients with various medical conditions, including illnesses, injuries, and chronic diseases.
  • Work collaboratively with a multidisciplinary team of healthcare professionals, including physicians, LPNs, CNAs, and aides, to ensure seamless patient care.
  • Conduct thorough assessments, document progress, and report changes to patients' physicians.
  • Develop and implement personalized care plans, educating patients and their families on medication management, therapy, diet, and exercise.
  • Coordinate discharge planning and ensure a smooth transition to other healthcare services as needed.
Requirements
  • Active Registered Nurse (RN) License in Colorado.
  • Graduate of an accredited school or program of nursing.
  • At least 1200 hours of experience as an RN.
  • Healthcare CPR/BLS Certification.
  • Home health care experience preferred.
  • Pass a background check and drug test.
