- Provide education and support to patients and their families to promote healthy outcomes and self-management.
- Document patient progress and outcomes in a timely and accurate manner.
- Current registration to practice as a Registered Professional Nurse in New York State.
- Minimum of one year of recent experience in maternal/child health.
- Demonstrated basic computer skills and proficiency in electronic medical records.
- Valid driver's license or NYS Non-Driver photo ID card.
